Control of Substances Hazardous to Health (COSHH) Assessment Form
COSHH forms are required for the substances that are used in the laboratory and they must be kept on file. These completed forms are indicated below and can be downloaded for review to ensure that you are aware of the issues associated with any of the substances that you are working with in the laboratory. Some COSHH forms have been completed for other substances not listed here and these may be obtained from the SLS pool, and these can be added here as the procedures are used in our laboratory.
